Click on the Forms tab above to find 2009-2010 forms. You will need to fill in and print the Membership Form, USAG Form, Acknowledgement Form, Locker Policy (Kempsville only). Please return all forms and attach check for $99 to cover dues and USAG number in the Secretary's box.
The Ad Book information letter is located under the Fundraising tab. The "Ad Book Information and contract" is a fillable and downloadable Acrobat file (PDF) which is located under the Forms page as "Ad Book contract" info. |
